John J. Rambo is a former United States Special Forces soldier who fought in Vietnam and won the Congressional Medal of Honor, but his time in Vietnam still haunts him. As he came to Hope, Washington to visit a friend, he was guided out of town by the Sheriff William Teasel who insults Rambo, but what Teasel does not know that his insult angered Rambo to the point where Rambo became violent and was arrested. As he was at the county jail being cleaned, he escapes and goes on a rampage through the forest to try to escape from the sheriffs who want to kill him. The Cahulawassee River valley in Northern Georgia is one of the last natural pristine areas of the state, which will soon change with the imminent building of a dam on the river, which in turn will flood much of the surrounding land. As such, four Atlanta city slickers - alpha male Lewis Medlock, generally even-keeled Ed Gentry, slightly condescending Bobby Trippe, and wide-eyed Drew Ballinger - decide to take a multi-day canoe trip on the river, with only Lewis and Ed having experience in outdoor life. They know going in that the area is ethno-culturally homogeneous and isolated, but don't understand the full extent of such until they arrive and see what they believe is the result of generations of inbreeding. Their relatively peaceful trip takes a turn for the worse when half way through they encounter a couple of hillbilly moonshiners. In Phoenix, the alcoholic and mediocre detective Ben Shockley is assigned by the Chief Commissary Blakelock to bring the witness Gus Mally from Las Vegas for a minor trial. Shockley travels to Vegas and finds that Gus Mally is an aggressive and intelligent prostitute with a college degree and she tells him that the odds are against her showing up in court. Shockley learns that she will actually testify against a powerful mobster and the mafia is chasing them trying to kill them both. He calls Blakelock and request a police escort from Phoenix to protect them. But soon he discovers that someone is betraying him in the police department. Now, Shockley and Malley hijack a bus and Shockley welds thick steel plates and transforms the cabin in an armored bus trying to reach the Forum. John Books an aging gunfighter goes to see a doctor he knows for a second opinion after another doctor told him he has a cancer which is terminal. The doctor confirms what the other said. He says Books has a month maybe two left. He takes a room in the boarding house and the son of the woman who runs it recognizes him and tells his mother who he is. She doesn't like his kind but when he tells her of his condition, she empathizes. Her son wants him to teach him how to use a gun. Books tries to tell him that killing is not something he wants to live with. Books, not wanting to go through the agony of dying from cancer, tries to find a quicker way to go. Together with Phil (from the first part) and unfortunately with Glen he tries to find the hidden gold of Curly's father in the desert of Arizona instead of attending a meeting in Las Vegas.
